The City of Santa Monica, Office of Sustainability Multi-Unit Dwelling EV Charging Station Pilot Rebate Program provides rebates to assist Santa Monica residents living in multi-unit dwellings in purchasing and installing electric vehicle service equipment (EVSE) on their properties.The Program may be modified without prior notice and terminated when the program goals are met, or funds are exhausted; whatever comes first. Applications are accepted on a first-come, first-served basis and processed in the order received. Under this program, there is a limit of one rebate application per property. The City of Santa Monica is not responsible for documents lost or destroyed in the mail via transit. Submitted rebate applications and accompanying documents become property of the City of Santa Monica. The City of Santa Monica may request additional documentation to verify that the business entity meets the criteria for the Program eligibility. The EV charging stations must be accessible to employees, customers, visitors, or tenants to charge EVs approved for highway application by the U.S. Department of Transportation. Program participants must be in compliance with all federal, state, county, city government regulations regarding the installation EV chargers, metering equipment, or any other support equipment required or necessary for the proper and safe installation of an EV charging station. In addition, the program participant has the sole responsibility of securing all necessary permissions from the property-owner, homeowner association, or any other organization from which permission to make such installations would be required. The program participant is solely responsible for the adherence to all applicable laws, codes, policies, conditions, covenants, and restrictions imposed by the aforementioned persons or organizations. All permits necessary must be obtained prior to the EV charging station installation. The City of Santa Monica makes no warranty and is not responsible for any representations, whether expressed or implied, included but not limited to the warranty of merchantability, fitness, performance, and longevity for any particular purpose, use, or application of the item(s) or measure(s), manufacturers, dealers, contractors, or any other third parties materials, workmanship, pollutants or any other matter with respect of the Program. Please allow up to 2 months after application submittal to receive the earned rebate check. By agreeing to the Program's terms and conditions, I agree to indemnify the City of any third-party claims. I have read and understand all the terms and contents of this document and understand what may or may not qualify for the Multi-Unit Dwelling EV Charging Pilot Rebate Program. The City of Santa Monica may apply additional restrictions or change program requirements. The City of Santa Monica requires on-site inspections to verify compliance with the Rebate Program terms. The City reserves the right to request and review receipts and invoices prior to final approval and issuance of rebates. The City reserves the right to inspect the installed charging unit during the final inspection appointment. Payment will not be processed until all required forms, tax documents, etc. are verified and approved. The City of Santa Monica may take and use photos of the property and project for reporting and marketing. All rights are reserved by the City of Santa Monica for all photography taken and used for reporting and marketing. I have read and understand that the City is complying with the Internal Revenue Service (IRS) and will be sending the Federal 1099 Tax forms to recipients who participate in our Multi-Unit Dwelling EV Charging Pilot Rebate Program. By applying for a rebate in this program, personal information listed on your application may be subject to disclosure to requesting parties pursuant to the California Public Records Act. The name on the W-9 must match the name on the rebate check. This form must be completed so that the Form 1099 can be completed in compliance with exemptions to the Federal Privacy Act of 1974, 42 UCS 405 (c )(2)(c ). Social Security numbers provided as part of the application process are held in confidence under terms of the Privacy Act and are not divulged or otherwise conveyed to individuals or organizations outside the Rebate Program. Under penalties of perjury, I certify that I am not subject to backup withholding because (a) I am exempt from backup withholding, or (b) I have not been notified by the Internal Revenue Service (IRS) that I am subject to backup withholding as a result of a failure to report all interest or dividends, or (c) the IRS has notified me that I am no longer subject to backup withholding.
